(1) Authors must register before June 22, 2004, to have their papers included in the proceedings (see also 2).
(2) Second and more papers to be presented by a registering author. Price is per additional paper.
(3) Student fee does not include: lunches, banquet and proceedings. Student registration cannot be used as the required registration for accepted paper.
